On November 23, 2013, Dan Meers came within inches of losing his life while practicing a bungee jump and zip line stunt at Arrowhead Stadium … home of the Kansas City Chiefs. What Meers anticipated as being the thrill of a lifetime ended up being the spill of a lifetime. The stunt went terribly wrong, and Meers plummeted 75 feet before crashing into the stadium seats. Miraculously, he survived. He spent nine days in the hospital and got some really big scars. He smiles when he says, “Scars are just tattoos that come with a cool story.” Meers will share his incredible story and the important lessons that he has learned about leadership and about life.

Each year more than half of nursing home residents experience a fall, while one in three residents experience multiple falls. These incidents can lead to serious harm to the resident, increase survey risks, and can be costly and time consuming for the facility. The presenters will simulate an interdisciplinary fall risk meeting demonstrating investigative techniques and strategies for understanding why the resident is falling while exploring innovative interventions to minimize falls or injury. Multiple case studies will be explored portraying examples across varied situations and resident characteristics. The simulation will model effective and efficient techniques facilitating team engagement throughout the process.

Who Gets to Decide? Identifying and Validating the Role and Authority of a Resident’s Legal Representative (NF, AL)

Residents have a right to make legal, healthcare, and personal decisions. In cases of reduced capacity, facility staff must often rely on a Resident Representative to make decisions for a resident. However, the scope of a Resident Representative’s legal authority is often unclear or not well defined. Mortier will assist facility staff in identifying the scope of authority of Resident Representatives. He will explore the distinctions between types of legal authority, including guardians, powers of attorney, and health care agents, and assist staff in spotting and evaluating discrepancies in the type of legal authority granted and the type of decision needing to be made.

Therapy professionals can be an invaluable support in certain areas of the MDS as well as areas impacting outcomes (quality measures) to reduce workload burden and enhance overall patient experience and well-being. Having a collaborative approach not only ensures more accurate and complete MDS assessments, but also strengthens the process for supportive documentation.

Discharge Disruption: Breaking Down Virginia’s Anticipated Updates to ALF Discharge Regulations (AL)

During its 2022 legislative session, the Virginia General Assembly enacted new requirements under the Virginia Code relating to discharges of assisted living facility residents. These requirements direct the Department of Social Services to implement regulations permitting facilities to discharge residents only under specific circumstances and subject to specific requirements. In this session, Kyle Renè will explore these requirements and what they mean for facilities. He will also discuss how facilities can best position themselves to address discharges once the new regulations go into effect.

The recent changes to the MDS have opened up a new avenue of assessments and engagement with our resident communities. Providers will need to understand where health equity and social determinants of health are addressed in the MDS and how embracing these concepts, how they are embedded in the SNF Quality Reporting Program and Value-Based Purchasing programs, and how implementation can enhance resident care experiences and outcomes. Health equity and social determinants of health are relatively new concepts that are just beginning to be identified in the LTC population through MDS reporting. SNF Value-Based Purchasing has expanded since its initial measurement metric of rehospitalizations. CMS has approved eight measures that when fully implemented in 2027 will have a significant effect on payment. If providers expect to retain any amount of the two percent withhold, they must embrace value based purchasing principles, including health equity. Providers will also gain an understanding of their health equity scorecards that have been posted in iQIES.
