AHCA is offering the second webinar in its two-part series for independent owners (IO) (providers with 10 or fewer skilled nursing facilities) to learn more about population health management (PHM) and how to engage in PHM. The next webinar is June 24 at 1:00 pm.

PHM is a discipline within health care that studies and delivers care across a specific group of people. It aims to improve the health and quality of life for a targeted group of patients. This is done through comprehensive care coordination, data analytics, risk stratification (high risk, low risk, rising risk), and interventions targeted at the individual, group, and community levels. Learn more about AHCA’s PHM initiatives.

This second webinar will focus on identifying PHM opportunities and taking action. Participants will hear from IOs currently involved in PHM provide insight on their reasons for participating, impacts to their core business, challenges, and successes.

From this webinar participants will be able to:

1. describe the experiences of IOs in PHM models.
2. explain how to identify opportunities for participation in their own markets and take action. 
3. discuss the potential benefits and challenges of IO participation in PHM.
