• You can download the file from our website. With this file, you can type in your Local Long-Term Care Ombudsman and Local Area Agency on Aging information.

Why it matters: Nursing facilities are required to share information about the protections under federal and state law that help nursing facility residents ensure they get the care and services they need.

• Surveyors may verify that facilities have posted updated information, particularly regarding residents’ rights.

Remember: Get the poster translated as needed based on your resident population
