Analysis of SMART data from 78 surveys conducted between December 1, 2017 and February 28, 2018 show that the top cited F-Tag is F-Tag 880-Infection Prevention and Control.  Among the 78 surveys, average number of deficiencies for standard surveys was 10.75; standard surveys with a complaint averaged 11.81 deficiencies.  Additional analysis includes the top ten cited deficiencies and severity. 

Over the 78 surveys, the following serious deficiencies were cited:

• 5 IJ (3 in the Richmond District)
• 19 G-Level (9 in Richmond District [1 facility received 4]; and 8 in Roanoke District);  
• 6 Substandard of Care

What is SMART and How You Can Use It

Beyond the publicly reported information, SMART (VCHA-VCAL’s Survey Management Analysis and Resource Tool) provides the severity and detailed descriptions for why the F-Tag was cited. The data in SMART can be further analyzed to learn the top five reasons and up to three additional sub-descriptions for a citation of a specific tag.
