This session is for providers in Virginia Health Planning Region 1-Northwestern.
VHCA-VCAL is launching an initiative to reduce preventable rehospitalizations by providing training on the INTERACT Quality Improvement Program. We will be hosting sessions in the five Virginia Health Planning Regions.
The regional training sessions are designed to bring together nursing facility staff and hospital representatives in the same area to help them better understand the services and capabilities each provider type offers.
VHCA-VCAL is proud to partner with Pathway Health and the Virginia Hospital and Healthcare Association (VHHA) to provide this training, which is funding by a grant from the Virginia Department of Medical Assistance Services using civil monetary penalty grant funds. The goal of this project is to achieve a combined 10 percent overall reduction in preventable rehospitalizations for individuals who reside in nursing facilities throughout Virginia by June 30, 2020.

Day 1 of each of the regional sessions will include a morning session for acute-care quality and safety leaders and an afternoon-session for nursing facility staff.
Day 2 is an intensive session for nursing facility staff who will be the readmissions prevention champions in their centers. Each Virginia nursing facility may designate one representative who will be the champion for her organization during the implementation process. Additional nursing center staff may register to attend for a fee of $196.
