As a reminder, DSS has published a new model form for Approval for Placement in Special Care Unit  (see 22 VAC 40-73-1100). 

DSS advises that while the form is intended to be helpful to providers, it is not required. The only required forms are the Disclosure Statement, the Liability Insurance Disclosure Notification Form. Forms are available on the DSS website; click on “Current ALF Providers” and then scroll down to “Forms.”
